8.1.2.2 NewDeclarativeEnvironment (E)
8.1.2.3 NewObjectEnvironment (O, E)
NewDeclarativeEnvironment and NewObjectEnvironment are never called with `E = null`, so the preamble shouldn't even state that this is possible.
