In the "absolutely final" draft, Annex B is listed in the TOC but it has a blanks title.
You probably have the wrong kind of heading item for it. I suggest you copy the Annex A header (including the title) into annex B and then edit it appropiately.

When I try that, the formatting is lost and no title is added to the TOC when I regenerate.

Fixed in the HTML version of 2nd edition: http://www.ecma-international.org/ecma-402/2.0/
And fix is confirmed in the draft for the 3rd Edition: https://tc39.github.io/ecma402/
